Requirements for Replacement of Lost Passport

• Duly accomplished passport Application Form
• Police Report with English translation
• Affidavit of Loss
• If possible, photocopy of the lost passport/Kuwait Civil ID
• Three (3) copies of recent picture (4.5 x 3.5 cm with plain white background)

  Note : There shall be a 15-day waiting period for passports not issued at the
  Embassy for verification and notification purposes.
