Requirements for Renewal of Passport

  • Personal Appearance (except below 8 years old and over 65 years old)
  • Duly accomplished passport Application Form  
  • Four (4) copies of recent picture (4.5 x 3.5 cm with royal blue background)
  • Philippine Passport and photocopy of pages 1 (data page), 2 & 3 (Limitations/Amendments), and 32 (inside back cover)
  • Philippine passport and photocopy of data page (page 1), Limitations and Amendments (pages 2 & 3), residence visa page and inside back cover (page 32)
  • Fee : KD 15.000

Requirements for Issuance of Passport and Registration of Birth to those Born in Kuwait

  • Personal appearance of the infant/child
  • Duly accomplished passport Application Form  
  • Four (4) copies of recent picture (4.5 x 3.5 c.m. with royal blue background)
  • Duly accomplished Report of Birth Form (in triplicate) with footprint of the infant at the back
  • Birth Certificate authenticated by the Kuwait Ministry of Foreign Affairs (with English translation) and three (3) photocopies
  • Marriage Certificate of parents and three (3) photocopies
    • If parents were married in the Philippines, the Marriage Certificate in SECPA (security paper) from NSO must be authenticated by the Department of Foreign Affairs - Manila.
    • If the marriage was solemnized by the Kuwaiti Ministry of Justice, the Marriage Certificate (with English translation) must be authenticated by the Ministry of Foreign Affairs and duly registered at the Embassy
  • Photocopy of parent’s passport (page 1 and residence visa page)
  • Personal appearance of either parent
  • Passport Fee : KD 15.000

Requirements for Replacement of Lost Passport

For woman who wants to adopt her married name

  • Marriage Certificate in SECPA (security paper) from NSO
    authenticated by the Department of Foreign Affairs – Manila
  • If the marriage was solemnized by the Kuwait Ministry of Justice,
    the Marriage Certificate (with English translation) must be
    authenticated by the Ministry of Foreign Affairs and duly registered
    at the Philippine Embassy.

Requirements for Solemnization of Marriages between Filipinos only

  • Photocopies of applicant's passports
  • Birth Certificates issued by the NSO and authenticated by Authentication Division, Department of Foreign Affairs (DFA)
  • NSO Certificate re: non-appearance of the applicant's name in the National Indices of Marriage or certificate of Single Status and Non-marriage issued by the NSO and authenticated by the DFA
  • Joint Affidavit of Single Status (form available)
  • Application for Marriage License (form available)
  • Affidavit of Parental Consent (if either applicant is 18-21 yrs.)
  • If either applicant is between 21-25 yrs., parental advice is needed. Parent/s of the applicant should inform the Embassy in writing. If such advice is not obtained or is unfavorable, the marriage license shall be issued 3 months following the publication of the application of the marriage license
  • Two pictures of each applicant
  • Notice of Marriage to be posted for 10 days
  • Fees: Solemnization of Marriage KD 18.000

Notarial Services

  • The Embassy, through its Consular Officers, provides notarial services for Filipinos who need to execute certificates and affidavits, both of public and private nature, that will be used in the Philippines. Generally, the requirements are proof of identify (and photocopy) and three (3) original copies of the executed affidavit/certificate
  • Fees: KD 7.500

Authentication Services

  • In compliance with the requirements of the Authentication Department of Kuwait's Ministry of Foreign Affairs, the Embassy will only process DFA-Manila Authentication Certificates with original documents that include birth certificates, marriage certificates, certificate of singleness, diplomas, transcripts of records, certifications, among others.
    The purpose of this is to avoid prolifieration of fake documents.

  • Request letter from sponsor/employer, including details of visas applicant and purpose of travel (for non-Kuwaitis)
  • Copy of round-trip airplane ticket
  • Personal appearance
  • Fees: KD 12.000 (single entry/3 months); KD: 24.000 (multiple entry/6 months); KD 36.000 (multiple entry/1 year)

Business/Recruitment Visa Requirements

  • Passport valid for at least six (6) months beyond intended length of stay in the Philippines
  • Photocopy of passport
  • Duly accomplished visa application form (download application form)
  • Two colored photos
  • Request letter from sponsor/employer, including details of visa applicant and purpose of travel (for non-Kuwaitis)
  • Copy of round-trip airplane ticket
  • Personal appearance (upon request)
    If the purpose of the visit is for recruitment of Filipino workers, applicant must submit the following:
  • Copy of the Special Power of Attorney verified by the Office of the Labor Attache
  • Copy of demand letter stating the number of workers to be hired, their positions, and salaries
  • Copy of approved block visa from the Ministry of Foreign Affairs
  • Fees: KD 12.000 (single entry/3 months); KD 24.000 (multiple entry/6months)
    KD 36.000 (multiple entry/1 year)

  • Authenticated secondary diploma and transcript of records
  • Personal History Statement (PHS) to be filled up in quadruplicate
  • Affidavit of support from parents
  • Police clearance
  • Medical examination report with life-size chest x-ray film
  • Certificate of good conduct
  • Duly filled up application form
  • Fees: KD60.000
